Attention parents: With the youth tackle season now underway, we will need your help in ensuring Union Rugby continues to offer a safe, healthy environment and follows the latest state protocols for COVID-19. Nearby are three documents that need to be completed by a parent or guardian for each player. Two forms only need to be completed once (Liability Release and Parent Questionnaire). The third (Daily pre-screening questions) must be completed each time your player comes to a practice or game.  All these forms can be brought to the next practice session and handed to a coach.
